Description of pharmacological action
Unoprost is a preparation containing phytosterol extract of the fruits of Serenoa repens, a low-growing fan-leaved sabal palm. The main active ingredients of the extract are phytosterols, fatty acids and fatty alcohols. Anti-inflammatory, antiexudative and antiandrogenic agent. Suppresses the synthesis of dihydrotestosterone (due to blockade of 5-alpha reductase types I and II) and its fixation to cytosolic receptors, which prevents the penetration of the hormone into the nucleus and leads to a decrease in protein synthesis. Reduces the formation of dihydrotestosterone, phospholipase A2 and arachidonic acid, reduces the synthesis of Pg, which regulate the tone and peristalsis of the lower urinary tract. Phytosterol extract interacts with cytosolic androgen receptors in prostate tissue through specific or competitive binding, preventing dihydrotestosterone from binding to these receptors. As a result, protein synthesis by cells decreases and the process of further hyperplasia of the connective tissue elements of the prostate is inhibited. Possible additional mechanisms of action include inhibition of prostate growth through prolactin binding to receptors in the prostate gland, an antiestrogenic effect, inhibition of epithelial cell proliferation, and inhibition of basal growth factor. The anti-inflammatory and anti-edematous effect of the drug is due to inhibition of the synthesis of prostaglandins, metabolites of 5-oxygenase and arachidonic acid in the prostate gland and active acid radicals in human neutrophils.
