Pharmacodynamics

Combined immunostimulating drug of bacterial origin for sublingual (sublingual) administration.

It is a polyvalent antigenic complex, including lysates of bacteria - the most common pathogens of respiratory infections. The lysates included in Ismigen® are obtained by mechanical action. Ismigen®, like other immunostimulants of bacterial origin of the second generation, has a nonspecific immunostimulating and specific vaccinating effect.

Ismigen®, sublingual tablets, has an effect on various parts of the immune system. Stimulates both local cellular and humoral immune responses and systemic immune responses. Activates phagocytosis, increases the content of lysozyme in saliva, and helps increase the number of immunocompetent cells.

Increases the functional and metabolic activity of macrophages (including alveolar) and polymorphonuclear leukocytes. Activates the processes of peroxidation and the expression of adhesion molecules on macrophage-monocyte cells and granulocytes LEA-1, MAC-1, p-150, ICAT-1, which ensures cooperation with T-lymphocytes and antigen-presenting cells and promotes the destruction of infectious agents. Stimulates the secretion of PGE2 by macrophage-monocyte cells, activates natural killer cells; stimulates the release of anti-inflammatory cytokines - IL-1, IL-2, IL-6, IL-8, interferon gamma, TNF-alpha; inhibits the synthesis of IL-4, IL-12; increases the concentration of secretory IgA in saliva, serum IgA, IgG, IgM; reduces the concentration of serum IgE.

As a result of treatment with Ismigen®, the frequency, severity and duration of acute respiratory tract infections are reduced; there is relief and leveling of symptoms such as fever, cough, shortness of breath, the need for antibacterial and anti-inflammatory therapy decreases; in patients with chronic diseases of the respiratory tract, the drug also prevents exacerbations.

Immunological and biological properties:

Ismigen is an immunostimulant based on bacterial lysate, which increases the body's resistance to infections by increasing the amount of serum and secretory antibodies, activating cellular and humoral factors of nonspecific immunity. This reduces the incidence and severity of respiratory infections and prevents the need for antibiotics.

Ismizhen has a dual mechanism of action

  • activates nonspecific resistance due to membrane antigens included in the drug, by activating dendritic cells, neutrophils, macrophages and NK cells; induce phagocytosis and cell lysis by stimulating the adhesion of macrophages to bacteria;
  • activates specific immunity by increasing the level of production of IL-2, specific serum IgA, IgG, IgM and slgA, activation of effector CD4 and CD8 T lymphocytes, activation of B lymphocytes.

Ismizhen is well absorbed in the oral mucosa.

The drug is used as part of complex treatment of the following diseases:

  • acute, subacute, recurrent or chronic infections and diseases of the upper respiratory tract (acute respiratory viral infections, influenza, rhinitis, including allergic pharyngitis, nasopharyngitis, tonsillitis, sinusitis, laryngitis, epiglotitis, sore throat)
  • acute, subacute, recurrent or chronic infections and diseases of the lower respiratory tract, including obstructive ones (tracheitis, tracheobronchitis, bronchitis, bronchiectasis, chronic obstructive pulmonary diseases (COPD)).

Thanks to its immunomodulatory effect, Ismizhen reduces the number and intensity of cases of acute respiratory diseases (ARI) in children who are often and long-term ill, and also reduces the number of exacerbations of chronic obstructive pulmonary disease (COPD) per year and the severity of their course. Ismizhen is used to treat infections resistant to antibiotic therapy and to treat complications of bacterial and viral infections. The drug can be used in combination with other drugs (antibiotics and mucolytic agents).

Contraindications

Hypersensitivity to the components of the drug.

Interaction with other drugs, including antibiotics, has not been noted.

Use during pregnancy or breastfeeding. Although toxic effects of the drug Ismizhen have not been observed in animals, it is recommended to avoid the use of the drug during pregnancy and lactation.

The drug does not affect the ability to drive a car or use other machinery.

Method of administration and dose.

For oral use in adults and children over 2 years of age.

Treatment of acute processes: dissolve 1 tablet under the tongue for 1-2 minutes once a day (you should refrain from eating for at least 30 minutes). Take daily for at least 10 days until symptoms disappear. The treatment course involves taking 10 tablets.

Preventive treatment: dissolve 1 tablet under the tongue for 1-2 minutes, 1 time per day for 10 days. After a 20-day break, repeat the 10-day course of taking the drug. The full course of preventive treatment, which achieves the maximum immunotherapeutic effect, is 3 months. During the full course you need to take 30 Ismizhen tablets. In other words, Ismizhen is taken in three ten-day courses over three months with two 20-day breaks between them. For young children, it is recommended to first crush the tablet, moisten it with boiled water at room temperature and put the resulting paste mixture in the mouth.

Children. The drug is used for children aged 2 years and older.

Overdose

No cases of overdose were observed.

Directions for use and doses

Under the tongue, take on an empty stomach. The tablet should be kept under the tongue until completely dissolved. Sublingual tablets should not be dissolved, chewed or swallowed.

Acute and subacute infections of the upper and lower respiratory tract - 1 tablet per day until the symptoms of the disease disappear (at least 10 days).

Recurrent infections of the upper and lower respiratory tract, prevention of exacerbations of chronic respiratory diseases - 1 tablet per day for 10 days. The preventive course includes 3 cycles of 10 days with 20-day intervals between them. A preventive course should be carried out no more than 1–2 times a year.

If you miss a regular dose, you should not double it the next time you take it.

Indications for use

The drug is prescribed for the treatment of influenza, rhinitis, pharyngitis, laryngitis, sinusitis, tonsillitis, tracheitis, bronchitis, obstructive pulmonary diseases .
