Picolax drops for oral administration 7.5 mg/ml 30 ml No. 1

Compound

Drops for oral use 0.75% contain the active component: sodium picosulfate - 7.5 mg/ml and additional substances: sorbitol, hydrochloric acid, sodium methylhydroxybenzoate and purified water.
One tablet contains the active substance: sodium picosulfate in terms of 100% anhydrous substance, 5 mg or 7.5 mg, as well as auxiliary components: lactose monohydrate, corn starch and pregelatinized 1500, magnesium stearate, colloidal anhydrous silicon dioxide.

Pharmacodynamics and pharmacokinetics

When treated with Picolax inside the body, its active component sodium picosulfate has a beneficial effect on the large intestine. Under the influence of bacterial flora, the drug is broken down, resulting in the formation of the active form - free diphenol.

Then the sensitive nerve endings of the intestinal mucosa are stimulated, enhancing its motility. Taking this drug does not lead to tenesmus and intestinal spasms. The development of the laxative effect occurs 6-12 hours after using one dose.

During the treatment of infants, insufficient effectiveness of the drug was noted, since they have a small amount of bacterial flora that produces sulfatases .

Pharmacological properties

Pharmacodynamics.
sodium picosulfate, the active ingredient of the drug picolax, is a local laxative of the triarylmethane group. sodium picosulfate, after bacterial breakdown in the colon, stimulates its mucous membrane, accelerating peristalsis, and promotes the accumulation of water and electrolytes in the lumen of the colon. the result is stimulation of defecation, reduction of transit time and softening of the feces. Pharmacokinetics. Following oral administration, sodium picosulfate reaches the large intestine without significant absorption. The active metabolite bis-(p-hydroxyphenyl)-pyridyl-2-methane is formed after bacterial breakdown in the colon; the onset of action of Picolax usually occurs 6–12 hours after dosing, depending on the release of the active metabolite. After oral administration, a small amount of the drug can be detected in the organs and systems of the body.

The laxative effect of the drug does not correlate with the level of the active metabolite in the blood plasma.

Indications for use

Picolax is prescribed to patients in the following cases:

  • atonic constipation caused by changes in diet, prolonged immobilization, severe general condition, fever , metabolic disorders and diseases of the cardiovascular system;
  • preparation for surgical and gynecological operations, as well as instrumental studies;

Pharmacological properties

Laxative medicine. The laxative effect develops after 6–12 hours.

Indications for use

For short-term use for constipation and diseases requiring relief of bowel movements.

Directions for use and doses

Adults. It is recommended to start taking the drug with a dose of 12-13 drops of Picolax® in water. If the effect is excessive, the dose must be reduced. If the laxative effect is insufficient, the dose can be increased. In cases of constipation, a dose of up to 24–32 drops of Picolax® (corresponding to 7.5–10 mg sodium picosulfate) in water is recommended. Children over 4 years of age should take 8–16 drops of Picolax® (corresponding to 2.5–5 mg sodium picosulfate) in water once a day. It is advisable to take Picolax® in the evening; the effect occurs the next morning. It is necessary to take enough fluid. Do not exceed the recommended dose. The optimal dose is the minimum dose sufficient to obtain the desired effect. Laxatives should be used as rarely as possible and for no more than seven days in a row. Prescribing laxatives for a longer period of time is possible only after a doctor has assessed the specific clinical situation.

Use during pregnancy and lactation

There are no clinical data on the use of sodium picosulfate during pregnancy. Since sodium picosulfate is a stimulant laxative, it is recommended for safety reasons not to use the drug during pregnancy. If you need to use the drug Picolax® during lactation, you should first consult with your doctor to assess the benefit/risk ratio.

Precautionary measures

Like all laxatives, Picolax® should not be prescribed daily or over a long period. Long-term or excessive use of the drug can lead to imbalance of fluid and electrolytes in the body and to hypokalemia. Patients taking Picolax® have reported dizziness and/or fainting. However, available information from such cases suggests that the fainting episodes were caused by straining to defecate or abdominal pain due to constipation, but were not associated with sodium picosulfate. Abuse of laxatives (private or long-term use or in excessive doses) can cause chronic diarrhea with subsequent loss of water, mineral salts (especially potassium) and other essential nutrients. In severe cases, dehydration or hypokalemia may develop, which can lead to cardiac dysfunction or neuromuscular diseases, especially in the case of concomitant use of cardiac glycosides, diuretics or corticosteroids. Abuse of laxatives, especially contact laxatives, can cause dependence (and therefore the need to gradually increase the dosage), chronic constipation and disruption of normal bowel function (intestinal atony). Treatment of chronic or recurrent constipation always requires consultation with a doctor for diagnosis, prescription of medications and monitoring during therapy. Consultation with a doctor is also necessary if there is a change in the frequency and other characteristics of bowel movements that last more than two weeks, and if there is no effect from the use of laxatives. Elderly patients or patients with underlying medical conditions should also consult a doctor before using the drug. The drug contains sorbitol and is not recommended for patients with hereditary fructose intolerance. Use in children The drug is contraindicated in children under 4 years of age. In children aged 4 to 12 years, the drug can be used only after consultation with a doctor. Effect of the drug on the ability to drive a car and use machinery. No studies have been conducted to assess the effect of the drug on the ability to drive a car or use machinery. Patients should be informed that they may experience dizziness or fainting during a parasympathetic vascular response (eg, abdominal cramps). If patients experience abdominal cramps, they should avoid potentially hazardous activities such as driving or operating machinery.

Interaction with other drugs

Broad-spectrum antibiotics, affecting the intestinal microflora, reduce the effectiveness of Picolax®. When used simultaneously with diuretics or corticosteroids, the likelihood of electrolyte disturbances increases. Electrolyte imbalance that occurs when taking Picolax® in large doses can lead to a deterioration in the tolerability of cardiac glycosides. Milk and antacids may alter the effect of the laxative: you should leave a gap of at least one hour before taking the laxative. Laxatives may reduce the absorption of other concomitantly taken orally administered drugs. The simultaneous use of laxatives and other medications is not recommended: after taking the drug, you must wait at least 2 hours before taking the laxative. Picolax® may increase the effect of oral anticoagulants and alter glucose tolerance.

Contraindications

Hypersensitivity to sodium picosulfate, triarylmethane derivatives or other components of the drug; narrowing of the intestinal lumen with impaired passage of intestinal masses, intestinal obstruction, acute abdominal conditions (for example, appendicitis); acute inflammatory diseases of the gastrointestinal tract; acute abdominal pain, accompanied by nausea and vomiting, as these symptoms may be manifestations of the above acute diseases; for diseases associated with water-electrolyte imbalance (for example, severe renal failure), the drug can only be used under medical supervision; children under 4 years of age; severe dehydration; pregnancy.

Compound

1 ml (24 drops) contains 7.5 mg of sodium picosulfate as an active ingredient. Excipients: sorbitol non-crystallizing solution, propylene glycol, purified water.

Overdose

Symptoms When taking the drug in large doses, watery stools (diarrhea), abdominal cramps, and clinically significant loss of potassium and other electrolytes may occur. When taking doses of the drug that are much higher than those recommended for normal constipation, ischemia of the colon mucosa may occur. As with any laxative, Picolax® may cause chronic diarrhea, abdominal pain, hypokalemia, secondary hyperaldosteronism and kidney stones when taken in high doses over a long period of time. Chronic and long-term use of laxatives causes renal tubular damage, metabolic alkalosis, and muscle weakness due to hypokalemia. Treatment In case of acute overdose within a short time after administration, absorption of the drug can be reduced or prevented by inducing vomiting or gastric lavage. If necessary, measures are taken to replace fluid losses and restore electrolyte balance. In some cases, it may make sense to use antispasmodics.

Side effect

The incidence of adverse reactions is indicated as: very often (? 1/10); often (from ? 1/100 to

Storage conditions

Store in a place protected from light at a temperature not exceeding 25 °C. Keep out of the reach of children.

Overdose

In case of an overdose of Picolax, patients may develop: diarrhea, pain in the abdomen, a decrease in the level of potassium in the blood and other disturbances in water and electrolyte balance.

Ischemia of the mucous membranes of the large intestine is also possible. If the drug is used in high dosages for a long time, patients may experience: chronic diarrhea, intestinal colic, hypokalemia , renal dysfunction, secondary hyperaldosteronism , urolithiasis and metabolic acidosis . Hypokalemia can lead to muscle weakness.

In such cases, gastric lavage, enterosorbents are usually prescribed, and further symptomatic therapy is carried out.

Reviews of Picolax

This drug is very popular among patients of various age groups. Therefore, reviews of Picolax can be found on medical forums where problems of digestion and gastrointestinal tract are discussed. In addition, users often say that they use this medicine to treat children. It is successfully used in the treatment of constipation during dysbacteriosis and other problems. Many people note its high effectiveness, which relieves intestines in a short time.

Quite often, this drug is used by patients suffering from chronic digestive disorders . Unfortunately, they often experiment with dosage or introduce additional agents. As a result, they develop side effects, mainly in the form of diarrhea. Therefore, experts recommend abandoning self-medication and strictly adhering to doctor’s prescriptions.

Picolax® (drops)

Features of application

Patients suffering from chronic constipation should undergo a complete diagnosis and determine the cause of constipation.
Prolonged and excessive use of the drug can lead to fluid and electrolyte imbalances and hypokalemia.

Discontinuation of Picolax may result in a return of symptoms. If Picolax has been used for chronic constipation for a long time, any return of symptoms may be more severe.

Cases of dizziness and/or syncope have been reported concurrently with the use of sodium picosulfate. Available information suggests that these phenomena are consistent with syncope during defecation (associated with the Valsalva maneuver) or are associated with a vasovagal response to abdominal pain.

Patients with rare hereditary fructose intolerance should not take this medicine because it contains sorbitol.

This medicinal product contains less than 1 mmol/dose sodium, which means it is practically sodium-free.

This medicinal product contains 2 mg sodium benzoate per 1 ml (30 mg sodium benzoate per 15 ml bottle, 60 mg sodium benzoate per 30 ml bottle).

Use during pregnancy or breastfeeding.

Pregnancy

No adequate clinical studies have been conducted in pregnant women. Animal studies have demonstrated reproductive toxicity of the drug when used in daily doses of 10 mg/kg and above. For safety reasons, Picolax should not be used during pregnancy if possible.

Breastfeeding period

Clinical data show that neither the active metabolite bis-(p-hydroxyphenyl)-pyridyl-2-methane (BHPM) nor its glucuronides are excreted in breast milk. Thus, Picolax can be used during breastfeeding.

Fertility

Clinical studies to evaluate the effect of the drug on fertility in humans have not been conducted.

Animal studies have shown no effect of the drug on fertility.

The ability to influence the reaction rate when driving vehicles or other mechanisms.

Studies of the effect on the ability to drive a car and operate other mechanisms have not been conducted. However, patients should be warned about the possibility of developing adverse reactions such as dizziness and/or syncope as a result of a vasovagal reaction (in particular abdominal spasm). If abdominal spasm occurs, the patient should avoid potentially hazardous activities such as driving or operating other machinery.
