pharmachologic effect

Foster has an anti-inflammatory effect, reduces the severity of symptoms of bronchial asthma and reduces the frequency of exacerbations of the disease, while having a lower incidence of side effects than systemic corticosteroids.

Indications

Basic therapy for bronchial asthma, involving the prescription of combination therapy (inhaled glucocorticosteroid and long-acting β 2 adrenergic agonist):

Patients whose symptoms are not sufficiently controlled by inhaled glucocorticosteroids and short-acting β 2 adrenergic agonists.

Patients receiving effective maintenance doses of inhaled glucocorticosteroids and long-acting β 2 adrenergic agonists.

Contraindications

Hypersensitivity to Foster's components, children under 12 years of age.

Carefully:

Pregnancy, lactation, pulmonary tuberculosis, fungal, viral or bacterial infections of the respiratory system, thyrotoxicosis, pheochromocytoma, diabetes mellitus, uncontrolled hypokalemia, idiopathic hypertrophic subaortic stenosis, third degree atrioventricular block, severe arterial hypertension, aneurysm of any location or other severe cardiovascular diseases (acute myocardial infarction, coronary heart disease, tachyarrhythmia, decompensated chronic heart failure, prolonged QTc interval (taking formoterol may cause prolongation of the QTc interval)).

Directions for use and doses

The dose of the drugs included in Foster is selected individually and depending on the severity of the disease. For adults and adolescents over 12 years of age: 1-2 inhalations twice a day. Patients should be under constant medical supervision to ensure that the dose of Foster is adequately adjusted. The dose should be reduced to the lowest dose that maintains optimal control of asthma symptoms. Once complete control over the symptoms of bronchial asthma is achieved on the background of the minimum recommended dose of the drug, at the next stage you can try prescribing monotherapy with inhaled glucocorticosteroids.

There is no need for special selection of the drug dose for elderly patients.

Side effects

Among the most common side effects associated with taking formoterol, those typical of beta2-agonists are described: hypokalemia, headache, tremor, palpitations, cough, muscle cramps, prolongation of the QTc interval. Side effects characteristic of beclomethasone dipropionate: candidiasis of the oral mucosa and pharynx, irritation in the throat.

Like other inhaled drugs, Foster can cause paradoxical bronchospasm.

Other side effects characteristic of formoterol: thrombocytopenia, angioedema, hyperglycemia, increased blood levels of insulin, free fatty acids, glycerol and ketone derivatives, sleep disturbances, hallucinations, fatigue, anxiety, changes in taste (dysgeusia), tachycardia, tachyarrhythmia, ventricular extrasystole, angina pectoris (coronary heart disease), atrial fibrillation, arterial hypertension, arterial hypotension, exacerbation of bronchial asthma, shortness of breath, nausea, itching, skin rash, urticaria, hyperhidrosis, myalgia, nephritis, peripheral edema.

special instructions

It is recommended to instruct the patient to rinse the mouth with water after inhaling maintenance doses in order to prevent the risk of developing candidiasis of the oral and pharyngeal mucosa.

The can is under pressure: do not expose it to high temperatures, do not pierce it, do not throw it into fire, even when empty. Use within 3 months from the start of use.

Drug interactions

β-adrenergic receptor blockers may weaken the effect of formoterol. Foster should not be prescribed simultaneously with beta-blockers (including eye drops), except in cases of emergency.

Storage conditions

In a place protected from light, at a temperature not exceeding 25°C.

Is it possible to overdose the drug?

If you use high doses of the drug, the side effects characteristic of formoterol may increase. A person may suffer:

  • pain in the head;
  • nausea;
  • gag reflexes;
  • trembling of hands and feet;
  • hypokalemia;
  • rapid heartbeat;
  • lethargy, loss of strength;
  • tachycardia;
  • an increase in blood glucose levels.

If these alarming symptoms appear, symptomatic therapy will be required. In especially severe cases, hospitalization is prescribed. In addition, it is necessary to strictly monitor the level of potassium in the blood.

An overdose of beclomethasone can lead to a deterioration in the function of the adrenal cortex. This condition does not require special therapy; it returns to normal on its own after a few days.

Against the background of regular use of high doses of aerosol, a systemic effect of hormones on the body is observed, which sooner or later will lead to a sympathoadrenal crisis (panic attack).

"Atrovent"

A bronchodilator, which is available in aerosol form. Manufacturer – Germany.

This Foster analogue blocks M-cholinergic receptors in the muscles of the tracheobronchial tract and suppresses the occurrence of reflex bronchoconstriction. Possessing structural similarity to the acetylcholine molecule, it is considered its competitive antagonist. The release of calcium ions is carried out with the help of mediators, including DAG (diacylglycerol) and ITP (inositol triphosphate). Anticholinergics prevent an increase in the intracellular level of calcium ions, which is observed as a result of the interaction of acetylcholine and muscarinic receptors located in the bronchial muscles.

The Foster aerosol analogue effectively prevents the occurrence of bronchospasm resulting from inhalation of cool air, cigarette smoke, the effects of various medications, and also directly eliminates bronchospasm caused by the influence of the vagus nerve. When used inhalation, it has virtually no resorptive effect. Bronchodilation, which develops after inhalation of this pharmacological drug, is mainly the result of its local and specific effect on the lungs, and not a consequence of its systemic effect.

We reviewed the instructions and analogues of the Foster inhaler. Reviews will be given below.

"Bekotid"

This medication is produced in France and is an inhaled glucocorticosteroid agent with anti-inflammatory activity. This drug is produced in the form of a dosed inhalation aerosol and belongs to the pharmacological category of GCS. The main element of the composition, beclomethasone dipropionate, is a prodrug with weak affinity for GCS receptors. Under the influence of esterases, it is transformed into an active metabolic substance - beclomethasone-17-monopropionate, which has a local anti-inflammatory effect. This analogue of the drug "Foster" helps reduce swelling of the bronchial epithelium and reduce the secretion of glands, reduces the number of mast cells in the bronchial membranes, and has a relaxing effect on smooth muscles.

"Salbutamol-Teva"

The most common Israeli-made inhalation medication, an analogue of Foster. The main active element is salbutamol sulfate, which effectively stimulates β2-adrenergic receptors. In therapeutic dosages, it acts on β2-adrenergic receptors of bronchial smooth muscles, providing a significant bronchodilator effect. This substance helps prevent and eliminate bronchospasm and increases lung capacity. In addition, it prevents the secretion of histamine, a slow-reacting component from mast cells, promotes dilation of the coronary arteries, and practically does not reduce blood pressure. This analogue of the Foster inhaler has a tocolytic effect: it reduces the tone and contractility of the myometrium and has a number of metabolic properties: it reduces the concentration of K+ in plasma, affects the processes of glycogenolysis and insulin secretion, gives a lipolytic and hyperglycemic effect, but increases the likelihood of developing acidosis.

The effect of the medication begins approximately 5 minutes after administration and lasts for 6 hours according to the instructions for use. Analogues of "Foster" should be selected by a doctor.

"Ingakort"

This is a German medicine available in the form of an inhaler. The active substance of the drug is flunisolide hemihydrate, which belongs to the category of local corticosteroids and, when administered in the correct doses by inhalation, has a pronounced anti-allergic and anti-inflammatory effect, which leads to a decrease in the severity of disease symptoms and the frequency of exacerbations of diseases that are accompanied by obstructive conditions of the respiratory tract (bronchial asthma, emphysema, chronic bronchitis).

Fluticasone propionate effectively suppresses the proliferation of mast cells, lymphocytes, neutrophils, macrophages, reduces the production and release of inflammatory mediators and other biologically active substances - prostaglandins, histamine, cytokines, leukotrienes.

This medication is contraindicated in cases of hypersensitivity, acute bronchospasm, status asthmaticus (as a first-line drug), and non-asthmatic bronchitis. This is confirmed by the instructions for use of the Foster inhaler analogue.

Using an inhaler

For effective treatment and to achieve the best absorption of the drug in the lungs, it is necessary to learn the correct technique for using the device.

Before the first inhalation or after a 3-day break in the already prescribed treatment, the first dose should be sprayed into the air to check the condition of the device. Holding the inhaler with the mouthpiece down, remove the cap, place the mouthpiece in your mouth and wrap your lips tightly around it. After exhaling through your nose, take a long, deep breath through your mouth while pressing the valve of the can. After this procedure, you should hold your breath and remove the device from your mouth.

"Flixotide"

This medication is a Russian analogue of the Foster inhaler. Fluticasone propionate (the main substance) belongs to the category of local corticosteroids and, when administered in the correct doses by inhalation, has a pronounced anti-allergic and anti-inflammatory effect, which leads to a decrease in the severity of disease symptoms and the frequency of exacerbations of diseases that are accompanied by obstruction of the respiratory tract (bronchial asthma, emphysema, chronic bronchitis).

Fluticasone propionate effectively suppresses the proliferation of mast cells, lymphocytes, eosinophils, neutrophils, macrophages, reduces the production and release of inflammatory mediators and other biologically active substances: prostaglandins, histamine, cytokines, leukotrienes. What other analogues of “Foster” are known?

Reviews

There are many reviews on medical websites about both the drug “Foster” and its analogues. Of particular note among them is the Russian analogue of this drug, Flixotide, which has a lower cost than other drugs and has a quick effect. As for the Foster inhaler, patients characterize it as a very effective remedy for eliminating an attack of bronchial asthma and in the development of obstructive bronchitis. It relieves symptoms quickly and rarely causes side effects.
