What is this?

According to official statistics, about 60% of people are infected with Demodex, but not everyone develops this parasitic disease. Mites of the genus Demodex are opportunistic pathogenic microorganisms, found in 90% of adults (in 72.5% of cases, Demodex of the eyelids is combined with lesions of the facial skin), but the disease does not occur in everyone. A mite measuring 0.2-0.5 mm lives in the ducts of the sebaceous and meibomian glands, in the mouths of hair follicles in humans and mammals. As a rule, the majority of the population is found to be asymptomatic carriers of Demodex mites. However, in conditions of reduced immunity, as well as under the influence of various negative external factors, such as exposure to high temperatures, solar radiation, unfavorable living and professional conditions, surgery on the eyeball and internal factors - nervous, vascular and endocrine diseases (for example, diabetes mellitus) systems, gastrointestinal tract and liver, metabolic disorders, the disease demodicosis occurs. Chronic demodicosis of the eyelids can cause conjunctivitis, keratoconjunctivitis and, as a consequence, MGD, which accompanies blepharitis - a lipid-deficient form of dry eye syndrome.

Diagnostics

Complaints: as a rule, patients complain of a burning sensation, itching in the eyelid area, aggravated by heat, redness of the eyelids, a feeling of “heaviness” of the eyelids, a feeling of discomfort and “dryness” of the eyes.

Biomicroscopy: the edges of the eyelids are thickened and hyperemic. A characteristic sign of blepharitis of demodectic etiology is the presence of “white muffs” or “collars” at the base of the eyelashes on glued eyelashes. Subsequently, severe inflammation of the ciliary edges of the eyelids develops, which become thickened and reddened. With a long-term chronic course, the edges of the eyelids become deformed, smoothing of the anterior and posterior ribs of the free edge of the eyelids occurs, which can result in trichiasis.

Laboratory examination of eyelashes using Demodex. The material for studying mites is epilated eyelashes, 3-4 from each eyelid, which are placed on a glass slide in a drop of glycerin, covered with a coverslip and microscoped. The diagnosis of demodicosis is made based on the detection of any phases of mite development during a microscopic examination of epilated eyelashes.

Tests for tear production and meibomian gland dysfunction. In order to diagnose dry eye syndrome and to decide on the advisability of prescribing tear replacement therapy, a Schirmer test is performed, a decrease in the indicators of which indicates a violation of the secretion of the aqueous phase of the tear film, as well as a compression test and a test for VRSP. Changes in the performance of these tests are a consequence of MGD and an indication for tear replacement therapy.

Treatment

Non-drug therapy

Eyelid hygiene is carried out to cleanse the surface of the eyelids, to relieve the inflammatory process and toxic-allergic reaction that occurs as a result of the activity of the mite. The course of treatment is 4-6 weeks; • antibiotics (tetracycline ointment, Tobrex, Tsipromed, etc.) are prescribed if there are signs of a bacterial infection. Course 7–10 days; • antiseptics (Vitabact, Okomistin, etc.) as a prevention of secondary infection. Course 7-10 days; • non-steroidal anti-inflammatory drugs (Diclo-f, Indocollir, etc.) – in the presence of signs of chronic blepharoconjunctivitis of non-bacterial etiology. Course 7-10 days; • antihistamines (Opatanol, Lecrolin, etc.) are prescribed if there are signs of an allergic and toxic-allergic reaction. Course 7–10 days; • tear substitutes (Hilo-Komod, Hilomax-Komod, Systane Ultra, etc.). Indications for their use are complaints of a feeling of “dryness” and discomfort in the eyes, MGD according to the compression test, a decrease in the Schirmer test and VRSP. Course 1–2 months.

It is recommended to consult a gastroenterologist, dermatologist, nutritionist (exclude spicy, salty, and sour foods). Limit visits to baths and saunas. Given the chronic nature of the disease, repeated courses of eyelid hygiene with the prescription of tear substitutes are recommended.

Steroid drugs are not used in the treatment of demodectic blepharitis, as they reduce local immunity and increase the number of mites.

When examining patients with demodectic blepharitis, it is very important to assess the condition of the facial skin for the prevalence of the demodectic process, as well as to identify signs of rosacea, a chronic recurrent disease of the facial skin, occurring mainly in women after menopause. The pathogenesis is based on angiotrophoneurotic disorders - in the zone of innervation of the trigeminal nerve, the tone of the superficial vessels of the skin, primarily the venous bed, changes. For the treatment of inflammatory processes in the facial area, it is recommended to use Demoten (Geltek-Medica, Russia), which contains a higher concentration of sulfur preparations than in Blefarogel 2.

Causes of demodicosis

As mentioned above, the main reason for the development of this disease is the active reproduction of the iron mite. There are several possible reasons that can lead to increased activity of these opportunistic microorganisms. These are internal and external factors. The first category includes:

  • A significant drop in immune defense and exhaustion of the body caused by poor nutrition, autoimmune pathologies, previous diseases, constant stress, etc.;
  • Viral and microbial diseases that occur in chronic forms (hepatitis, tuberculosis, etc.);
  • Helminthiases. The vital activity of helminths contributes to the weakening of the immune system;
  • Malignant neoplasms. With many types of oncology, the body becomes exhausted, making it more difficult for it to cope with infections;
