In recent years, methods of correction and stimulation of immunity have been actively introduced into clinical practice. Immune system disorders can manifest as hyperimmune responses, primary or secondary immune deficiencies, and various patterns of system dysfunction. Immunostimulating agents (often called immunomodulators) are drugs that activate cellular and humoral immunity, the phagocytic function of macrophages, the release of interferons, and also increase the nonspecific resistance of the body. They are used mainly to treat diseases caused by primary or secondary deficiency of the immune system or its dysfunction. Among the immunomodulators, there are drugs of microbial origin (prodigiosan, BCG vaccine), peptide (interferon-a, interferon-b, interferon-g, etc.) and synthetic (levamisole, non-steroidal anabolic agents such as methyluracil, Trecresan, etc.) and herbal immunomodulators ( preparations from Echinacea purpurea, licorice root, spirullina algae, etc.) [2]. All immunomodulators find their use in medical practice, since they have their own characteristics of impact.
New immunomodulators include the drug Trecrezan, which recently appeared on the pharmaceutical market [3]. Trecrezan is a highly effective drug with a pronounced immunostimulating and adaptogenic therapeutic effect. Trekrezan is an original drug, developed by the Irkutsk Institute of Organic Chemistry of the Russian Academy of Sciences (academician Voronkov M.G.), its tests were carried out at the Military Medical Academy and the Influenza Institute of the Russian Academy of Medical Sciences. Trecrezan is non-toxic (LD50 for rats > 3700 mg/kg with intraperitoneal and > 6500 mg/kg with oral administration of the drug), has a stress-protective effect in models of immobilization and painful hypodynamic stress, has the ability to accelerate the repair of damaged tissues (liver, myocardium, muscles) , protects internal organs from the damaging effects of toxins, microwave irradiation, and infectious factors. The drug has pronounced antioxidant activity and immunostimulating properties [3]. Let's consider some features of the immunomodulatory effect of Trekrezan. The effect of Trecrezan on indicators of nonspecific protection and the state of the immune system. In an experiment on mice, Trecrezan (20 mg/kg) when administered intraperitoneally for 6 days did not affect the weight of the thymus, but caused a slight increase (+20%) in the weight of the spleen. Trecrezan had no effect on the cell content of peritoneal exudate. The percentage of macrophages among the cells of peritoneal exudate did not change in the groups and remained within the normal range - about 30%. The percentage of T-lymphocytes in the spleen under the influence of Trecrezan increased slightly (+18%). In the control group, the content of T- and B-lymphocytes did not change. A stimulating effect of Trecrezan on resident peritoneal macrophages was noted. At the same time, a twofold increase in both spontaneous and induced ability of macrophages to restore the absorption of nitroblue tetrazolium (NBT) and also to absorb neutral red was noted. A study of the effect of the drug on the functional activity of peptone-induced neutrophils showed that Trecrezan has the ability to increase chemotactic activity by 2 times and phagocytosis of staphylococci by 80%. Thus, Trecrezan, when administered intraperitoneally for 6 days, caused an increase in the percentage of T-lymphocytes in the spleen in mice, had a stimulating effect on the functions of peritoneal phagocytes, increasing phagocytic activity and chemotaxis of neutrophils, as well as enhancing the “oxidative burst” of macrophages and their absorption capacity , activated cellular and humoral immunity, increasing phagocytosis and chemotaxis of neutrophils both in intact animals and in secondary immunodeficiency (modeled immunodeficiency). The effect of Trecrezan on the production of serum a- and g-interferon. The study was carried out in accordance with the guidelines for studying the specific activity of interferons and interferon inducers [6] at the Influenza Research Institute of the Russian Academy of Medical Sciences and the Research Institute of Children's Infections of the Russian Federal Healthcare Service (St. Petersburg) (Table 1). Evaluation of the ability of the drug Trecrezan to induce serum interferon in experiments showed that it has interferonogenic activity. In the first hours after the administration of Trecrezan, a-type interferon is produced, which is subsequently replaced by g-interferon within 24 hours. Considering the identified interferonogenic activity of Trecrezan, we can count on expanding the scope of its use. A special feature of Trekrezan's action is its ability to induce g-interferon, which has a much wider range of immunomodulatory effects compared to other types of interferons. The experimental data presented show that Trekrezan has high immunotropic activity. It stimulates all types of immunity - cellular, humoral, phagocytosis. An important feature of the action of Trecrezan is its interferonogenic activity against interferon-g, which is a rather rare phenomenon. The drug is indicated to increase immunity, strengthen the body's defenses; as an adjuvant for long-term antibiotic therapy of chronic infectious diseases. Apparently, Trekrezan can be used in the complex treatment of hepatitis B and C, as well as HIV infection. In addition, the stress-protective and antiasthenic properties of Trekrezan, along with its ability to reduce the toxic effects of many xenobiotics (including drugs), allow Trekrezan to be positioned as a modern effective immunomodulator with broad adaptogenic properties.
