pharmachologic effect

Docosanol is an antiviral agent. The exact mechanism of antiviral action is unclear. In vitro studies have shown that docosanol affects the adhesion of the virus to the plasma membrane, which inhibits viral entry into the cell and its replication. In vivo studies have shown that cells treated with docosanol are resistant to infection by lipid-enveloped viruses such as Herpes simplex virus 1. Docosanol has no effect against non-enveloped viruses.

Docosanol 10% cream has been studied in clinical trials compared to placebo (containing polyethylene glycol). A total of 737 patients were randomized. The mean time to complete recovery was 4.1 days in the docosanol group and 4.8 days in the placebo group, with a difference of 17.5 hours (0.7 days).

Erazaban cream 10% 2g

Compound

Active substance: docosanol - 100 mg. Excipients: sucrose, almond oil, propylene glycol, benzyl alcohol, water.

Pharmacokinetics

Docosanol is metabolized intracellularly to docosanic acid (the main metabolite). Docosanol and its metabolite are endogenous components of cell membranes in humans, especially in red blood cells, brain, lungs and kidneys.

When applying the cream on intact skin: absorption is minimal, not detected in the blood. Affected skin: absorption is moderate. Excreted by the kidneys.

Indications for use

Infections of the skin and mucous membranes caused by the Herpes simplex virus type 1, incl. herpes lips

Contraindications

  • hypersensitivity to docosanol or any other component of the drug;
  • age up to 12 years (no experience of use);
  • patients with rare diseases: fructose intolerance and glucose/galactose malabsorption syndrome or sucrase/isomaltase deficiency.

Carefully:

pregnancy and lactation period.

Directions for use and doses

Externally. It is recommended to apply the drug 5 times a day (approximately every 3 hours during the day) as soon as possible after the onset of infection, apply a thin layer to the affected and adjacent areas of the skin.

It is important to begin treatment for a recurrent infection during the prodromal phase or at the very beginning of the infection.

The duration of treatment is at least 5 days. If there is no healing, treatment can be continued for up to 10 days. If symptoms persist for more than 10 days, you should consult a doctor.

No dose adjustment is required in elderly patients or patients with impaired renal function.

The cream is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas.

Storage conditions

Store out of the reach of children at a temperature not exceeding 25°C.

Best before date

3 years.
After the expiration date, do not use the drug. After first opening, store for no more than 6 months.

special instructions

Avoid contact of the drug with the eyes; do not apply to the skin near the eyes. To achieve the maximum therapeutic effect, it is necessary to use the drug at the first signs of the disease (burning, itching, tingling, feeling of tension and redness).

Patients with reduced immunity (with HIV/AIDS or after bone marrow transplantation) should follow the doctor's recommendations when treating any infectious diseases.

Side effects

The frequency of adverse reactions that may occur during therapy is given in the following gradation: very often (≥10); often (from ≥ 1/100 to <1/10); uncommon (≥1/1000 to <1/100); rare (from ≥1/10,000 to <1/1000); very rare (<1/10,000), including isolated cases.

From the central nervous system: very often - headache.

Locally: often - dry skin, itching, rash, short-term redness, soreness, peeling, burning or tingling in areas where the drug was applied, vulvitis.

There are isolated reports of cases of angioedema with external use of the drug.

If any of the side effects indicated in the instructions get worse, or you notice any other side effects not listed in the instructions, tell your doctor.

Use during pregnancy and breastfeeding

Adequate and strictly controlled clinical studies of the safety of the drug during pregnancy have not been conducted. Experimental studies have not revealed teratogenic and embryotoxic effects on the fetus.

The use of Erazaban is indicated only when the expected benefit to the mother outweighs the potential risk to the fetus and/or child.

If it is necessary to use the drug Erazaban during lactation, it is recommended to stop breastfeeding.

Interaction

When used externally, no interactions with other drugs were detected.

An enhanced effect is observed with the simultaneous administration of immunostimulants.

pharmachologic effect

Pharmacodynamics

Docosanol is an antiviral agent. In vitro studies have shown that docosanol affects the adhesion of the virus to the plasma membrane, which inhibits viral entry into the cell and its replication. This mechanism of action makes the emergence of Herpes simplex strains resistant to the antiviral effects of docosanol unlikely, and also protects against the penetration of the virus into healthy cells. In vitro studies have shown that cells treated with docosanol are resistant to infection by lipid enveloped viruses such as Herpes simplex virus type 1.

Pharmacokinetics

When used externally, the drug is practically not subject to systemic absorption. Docosanol is metabolized intracellularly to docosanic acid (the main metabolite). Docosanol and its metabolite are endogenous components of cell membranes in humans. When applying the cream on intact (healthy) skin: absorption is minimal and not detected in the blood. When used on affected skin: absorption is moderate. Excreted by the kidneys.

Indications

Infections of the skin and mucous membranes caused by the Herpes simplex virus type 1 (including herpes labialis).

Contraindications

Hypersensitivity to docosanol or any other component of the drug;

age up to 12 years (no experience of use).

Carefully:

pregnancy and lactation period.

Directions for use and doses

Externally. It is recommended to apply the drug 5 times a day (approximately every 3 hours during the daytime) as soon as possible after the onset of infection, apply a thin layer to the affected and adjacent areas of the skin. The average recovery time is 4 days.

The cream is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas.

The effectiveness of the therapy depends on the time of initiation of therapy: the sooner therapy is started from the onset of symptoms of the disease, the higher the effectiveness. If treatment for a recurrent infection is started during the prodromal phase or at the very beginning, Erazaban prevents the appearance of blisters and crusting. If blisters have already appeared, it reduces swelling and pain; after 2 days, pain, burning, itching, tingling disappear, and visible herpetic manifestations - blisters on the lips or on the skin at the site of the lesion - are significantly reduced.

It is necessary to carry out the full course of therapy for 5 days in order to achieve the greatest effect from the therapy, even if signs of infection disappear after 1-2 days of treatment. If there is no healing, treatment can be continued for up to 10 days.

No dose adjustment is required in elderly patients or patients with impaired renal function.

Use during pregnancy and breastfeeding

Experimental studies have not revealed teratogenic and embryotoxic effects on the fetus.

Before using Erazaban during pregnancy or breastfeeding, consult a doctor.

Side effects

Headache, dry skin, itching, rash, short-term redness, soreness, peeling, burning or tingling in the areas where the drug was applied. These side effects, as a rule, do not require discontinuation of the drug.

There are isolated reports of cases of angioedema with external use of the drug.

If any of the side effects indicated in the instructions get worse, or you notice any other side effects not listed in the instructions, tell your doctor.

ERAZABAN (cream)

(who sits on a percentage of medical representatives, of course) said that if I buy this product, I will never again have the problem of a cold on my lips.
I immediately agreed to the purchase, as a sensitive and trusting person. The packaging is cardboard, red, white and yellow.

Made in Great Britain. Available without a prescription.

The cost of the medicine is high, about 500 rubles for 2 grams of product.

The shelf life is 3 years, it is indicated on the side of the box, as is the composition.

The tube is quite small.

The tube is not sealed, I found out in unpleasant conditions. I decided that I needed to take a photo for the review, of the sealed tube. I take it out of the box, unscrew the lid and suddenly the cream starts to come out. The only explanation I gave for this was that it was hot in the city and the cream could have overheated while I was driving from the pharmacy.

I went to get the saucer, and after that I was no longer afraid to remove the lid. The cream didn't come out all, but it all looked sad. The tube was not hot, and I was on the subway, where there is no heat. I can’t explain why this happened.

Having removed the excess cream onto a plate, I untwisted the sealing point of the tube and found that it unwinds quite easily, but sometimes the edge is tightly sealed and it is impossible to unroll anything. That's how much cream came out. I then used it for 2 days, by the way.

The color of the cream is uniform, white. The smell is medical, ordinary.

So, taking the tube by the sides, I pressed a little and tried to pull the cream back in, due to the vacuum that had arisen inside. I tried this number with baby cream, but it didn’t work. I had to leave some of the Erazaban on the plate, cover it with a bag and put it in the refrigerator. I screwed the tube back on.

I decided that the cream had overheated, so it came out, but it turned out that it behaves this way all the time, albeit with less expressiveness. That is, I take out the cream, squeeze it out a little, while I smear it on my lip, it continues to come out. The nose of the cream is very thin, and in the end, the consumption of the cream turned out to be very large.

The instructions are printed on both sides of a small piece of paper.

If you click on the photo, it will enlarge and you can read the entire text.

Active ingredient: docosanol.

Contraindication is sensitivity to the drug. You will have to try and stop if any side effects occur. The list of side effects is not long, but it is formidable.

It is important to know that the cream is produced not only in two-gram tubes, but also in larger ones. If you often suffer from this scourge, then you can buy a larger package.

The instructions suggest using Erazaban 5 times a day. Recovery time is 4-6 days.

When I first felt that something was hurting on my lip, I started using Acyclovir. It was this morning, at 9 am. By lunchtime I bought Erazaban and began treatment with it. By this time there were already two small bubbles on the lip. By the evening I had a huge bubble on my lip, but I could talk, eat, smile calmly, nothing particularly hurt.

It must be said that Acyclovir looks much larger and this makes Erazaban seem more effective. I thought that such a tiny tube contained a concentrated medicine that would guarantee a quick recovery.

There was no sensation when using it. If Acyclovir stings and then spreads over the lip, then nothing stung or hurt. The cream dissolves but does not go beyond the lips. Of course, it all doesn’t look very good, but I was lucky that the peak of the disease occurred on the weekend that I spent at the dacha.

Overnight the bubble deflated and on the second day I had such a soft crust on my lip, a swollen lip underneath, but nothing hurt either. I even put on lipstick for one hour because I was meeting people for business.

On the third day, the cold looked even more modest - it was a dried crust on the lip. I was sitting, eating porridge, drinking tea, and suddenly I felt something was wrong. I touch my lip with my finger, and the crust falls off. How is that? After that, everything began in a circle. The skin healed, but again there was pus, blood, everything hurt. The medicine stopped helping, and I also began to feel that my upper lip was starting to hurt.

On the 6th day I stopped using Erazaban and started Zovirax. During this time, I used up almost all of the drug. Do you see how skinny the tube is now?

Now I will show you photographs of the lip during the treatment process. On the top there is just this large and dense crust that fell off from warm and hot food; The bottom photo shows the current state. Until now, the lip has some sensitivity to salty, sour and hot foods. I apply Zovirax 3 times a day, and I don’t feel like going back to Erazaban.

My impressions

The total duration of my treatment was 10 days, of which I was treated for 6 days with Erazaban and 4 days with Zovirax. Erazaban is an effective medicine, but not completely effective. I directly felt the effect - everything immediately stopped hurting, I could talk and eat calmly, it healed perfectly while you sat covered in cream. And it itself does not spread, is not bitter, not greasy. Everything is fine, but the herpes does not go away. And for some reason the cream is always cramped in the tube. And the medicine is too expensive. There are only three drawbacks to this drug, but you will agree that they are significant.

Let me remind you that the instructions promise a quick recovery in 4-6 days, which is why I stopped treatment with this drug on the 6th day. I decided that this was not my case.

I'll show you Zovirax and note that it contains not only Acyclovir, but also Hydrocortisone. It has a very tiny consumption, I like this drug.

Maybe Zovirax could not have done it on its own in four days, but lay on the prepared soil and that’s why it helped so much. Maybe the presence of Hydrocortisone helped, but I decided to stay with this drug for now, although you can see that it is also expired, but it was what it was at the dacha.

Price comparison: - Acyclovir costs 80 rubles (10 grams), - Erazaban costs 490 rubles (2 grams), - Zovirax Duo-Active costs 350 rubles (2 grams).

It’s difficult for me to draw a medical conclusion, I hope this review will help with choosing a medicine. I can’t take off all the stars, I’ll leave a few. I recommend this drug as an addition to others or if you have tried everything else. It is not worth starting treatment with this expensive drug.

Thank you for your attention, don't worry!
