Pharmacological properties:
Tiotropium bromide is a long-acting antimuscarinic agent. It has equal affinity for the M1–M5 muscarinic receptor subtypes. As a result of inhibition of M3 receptors, smooth muscles relax. The bronchodilating effect lasts for at least a day and depends on the dose. The prolonged effect of the drug is apparently explained by the very slow dissociation of the substance from M3 receptors; in comparison with ipratropium bromide, its half-dissociation period is much longer. When administered by inhalation, tiotropium bromide has a selective effect on the bronchi without causing systemic side effects of m-anticholinergic drugs. Compared to M3 receptors, M2 receptors dissociate faster.
Indications for use:
Inhalations with Spiriva Respimat solution are recommended for maintenance therapy in order to improve the quality of life and reduce the frequency of exacerbations in the following diseases and conditions: COPD, chronic bronchitis, emphysema (with persistent shortness of breath); bronchial asthma with persistent symptoms of the disease during therapy with at least inhaled glucocorticosteroids (GCS) to reduce its manifestations (as additional maintenance therapy).
Mode of application:
The Spiriva Respimat inhaler is intended for use once a day at the same time. For each daily use, 2 inhalations should be taken. The number of inhalation doses is 60, which corresponds to 30 therapeutic doses, subject to the conditions of use. For bronchial asthma, the effect of the drug occurs after a few days of its use. If the inhaler has not been used for more than 7 days, before using it for the first time, you should point it into the air and press the dose button. It is recommended to clean the mouthpiece, including its metal part, with a damp cloth at least once every 7 days. The dose indicator shows approximately how many doses are left; when it reaches the red area of the scale, this will mean that the remaining drug will last for about 7 days, provided that it is used according to the instructions. When the dose indicator reaches the end of the scale, the inhaler will lock. This means that the drug has run out. Preparation for the first use of the Spiriva Respimat inhaler: Remove the transparent sleeve (keep the cap closed) by pressing the lock button and pulling firmly on the transparent sleeve with the other hand. Insert the cartridge with the narrow end into the inhaler, place the bottom of the inhaler on a hard surface and press it firmly until it clicks, this will mean that the cartridge is in place. Place the transparent sleeve in place until it clicks. Keeping the cap closed, turn the transparent sleeve in the direction indicated on the label half a turn until it clicks. Open the cap all the way. Pointing the inhaler down, press the dose button, then close the cap. Repeat steps 4–6 until a cloud of aerosol appears, then repeat them 3 more times (steps 4–6 must be performed both when using the inhaler for the first time and when it has not been used for more than 3 weeks). Daily use of the Spiriva Respimat inhaler: Turn the transparent sleeve in the direction indicated on the label until it clicks with the cap closed. Open the cap all the way. Exhale slowly, fully, wrap your lips around the mouthpiece without blocking the air intakes, press the dose button while taking a deep breath, hold your breath for 10 seconds or as long as possible. Repeat these steps for the second dose.

The drugs Spiriva and Spiriva Respimat are not used to treat bronchospasm , especially during acute attacks.
