Pharmacological properties of the drug Ultraproct

Fluocortolone has anti-inflammatory, antiallergic and antipruritic effects. As a result of the drug's action, capillary dilatation, intercellular edema and tissue infiltration are reduced, and capillary proliferation is inhibited. The drug Ultraproct contains two fluocortolone esters, the onset of the main action of which occurs at different times, which allows you to achieve a quick effect and maintain it for a long time (two-phase action). Cinchocaine as a local anesthetic reduces pain. Due to the different lipophilicity and molecular weight of fluocortolone pivalate and fluocortolone caproate diffuse at different rates into the site of inflammation, which leads, on the one hand, to a rapid onset of action, and on the other, to an increase in the duration of action. It is assumed that after topical application, both esters are hydrolyzed to free fluocortolone and corresponding acids at the level of the perianal skin or rectal mucosa. Studies of formulations similar to Ultraproct have shown that less than 15% of the applied dose of fluocortolone pivalate is absorbed rectally. Adsorbed fluocortolone is metabolized in the liver, most metabolites are excreted in the urine. Like corticosteroids, cinchocaine has a local analgesic effect. Analgesic effective plasma levels of cinchocaine are not a necessary prerequisite. According to assumptions, the adsorbed dose of cinchocaine is small to detect side effects if Ultraproct is used according to the instructions. After absorption, cinchocaine is biotransformed into a certain number of metabolites. Even under the assumption of complete adsorption, systemic effects can be excluded when using both forms of the drug according to the instructions due to the low dosage of the active ingredients of the drug.

Fluocortolone

Pharmacokinetics

Use during pregnancy and breastfeeding

A number of epidemiological studies suggest a possible increased risk of developing cleft palate in newborns whose mothers received oral corticosteroids in the first trimester of pregnancy. There is insufficient data on the use of local corticosteroids during pregnancy, but in this case the likelihood of adverse effects is very low due to the minimal bioavailability of corticosteroids when applied topically.

Ultraproct ointment and suppositories should be prescribed with caution to pregnant women in the 2nd and 3rd trimester. When prescribing to pregnant and lactating women, it is necessary to compare the expected benefits of treatment for the mother with the possible risk to the fetus and infant.

When prescribed by a doctor during pregnancy and lactation, the drug should be used for a short time.

Contraindications

The use of Ultraproct is contraindicated in:

  • manifestations of syphilis , tuberculosis in the area where the drug is used;

Directions for use and doses

Ultraproct is recommended for use after defecation. Before using the drug, you should perform hygiene of the anus (anus).

Rectal ointment

Squeeze a small amount of ointment (about the size of a pea) onto your finger and lubricate the area around the anus and inside the anal ring. Protruding hemorrhoids should be generously lubricated with ointment, gently pushing them back with your finger.

If the suppositories have softened under the influence of heat, they should be placed in cold water without removing the packaging, then they will return to their previous shape.

There is usually rapid improvement, but this should not be a reason to stop treatment too soon. To avoid relapses, you should continue to use Ultraproct for at least 1 more week, although with less frequency (ointment - 1 time per day or 1 supp. - every other day), even with complete disappearance of symptoms. However, the duration of treatment should not exceed 4 weeks.

Overdose
