Evaluation of the effect of Cardionat on the effectiveness of antianginal therapy and the functional state of the myocardium in patients with coronary artery disease in combination with arterial hypertension associated with type 2 diabetes mellitus

Pharmacodynamics and pharmacokinetics

The effect of the medicine is due to the characteristics of its components.

Carnitine is an anabolic . It is broken down in the liver, central nervous system and kidneys with the participation of iron and ascorbic acid. In plasma it is found in free form or in the form of acylcarnitine esters . It is the main regulator of fatty acid metabolism in the myocardium, liver and skeletal muscles. Helps in the release of toxins and metabolites , as well as in improving metabolic processes. With oxygen supply to cells at a normal level, it transfers metabolism to the oxidation of fatty acids, and under hypoxic conditions it removes their toxins from mitochondria, transferring metabolic processes to the oxidation of glucose, thus producing an antihypoxic effect .

Carnitine improves performance, increases muscle mass, reduces the effect of thyroid hormones , accelerates growth, promotes weight loss and normalizes metabolism in hyperthyroidism . It also reduces the symptoms of mental and physical fatigue, has an anti-ischemic effect , and normalizes the transmission of nerve impulses by activating the synthesis of choline , glutamic acid and acetylcholine . The product reduces the ammonia content in nerve tissues and has neuro- , cardio- and hepatoprotective effects . In addition, Carnitine affects the limitation of the infarct zone and increases cellular immunity . It eliminates problems associated with the functioning of the nervous system in people with chronic intoxication, prevents dystrophic changes in the retina, and mitigates the manifestations of withdrawal syndrome . Acts as an anti-ketogenic agent , reducing the production of ketone bodies from higher fatty acids.

Cobamamide is characterized by anabolic activity , takes part in the synthesis of labile methyl groups sulfhydryl groups in erythrocytes , has a positive effect on metabolic processes, the formation of choline , methionine , nucleic acids and creatine . It also improves bone marrow function, helps normalize the functioning of the nervous system and liver, activates the blood coagulation system, has an anti-ketogenic effect , and in high doses increases the activity of thromboplastin and prothrombin .

Pyridoxal 5-phosphate affects metabolism, as well as the functioning of the nervous system. It is a coenzyme of many enzymes that play an important role in non-oxidative amino acid . In addition, it is involved in the metabolic processes of tryptophan , cysteine , methionine , glutamic and other amino acids . histamine metabolism , increases the concentration of glycogen in the liver and normalizes its detoxifying properties, helps improve fat metabolism, and catabolizes neuromuscular processes.

Lysine is an important amino acid , which is a component of the processes of assimilation and growth, activates mitosis , acts as an antiviral agent against herpes , helps in ossification and growth of bone tissue, normalizes oogenesis and spermatogenesis .

Cocarboxylase affects metabolism. In addition, it reduces the content of lactic and pyruvic acids in the body, promotes the trophism of nerve tissue, increases the absorption of glucose, and has a cardioprotective effect .

Pharmacological effects

It is Meldonium that gives drugs the following therapeutic effects:

  • Angioprotective. The drugs normalize the functionality of vessel walls and improve microcirculation. Even small capillaries have a positive effect by restoring their tone. This effect leads to the establishment of metabolic processes and the elimination of edema.
  • Anganginal. The drugs can eliminate coronary insufficiency, including stopping attacks of angina. Medicines help reduce the need for oxygen in the heart muscle, while simultaneously increasing the delivery of a vital compound to the myocardium.
  • Antihypoxic. Helps increase the body's resistance to oxygen deficiency caused by both external factors and various diseases.
  • Cardioprotective. Medicines correct and restore the functional state of the heart muscle.

The therapeutic effects of Cardionate and Mildronate are due to the active ingredient Meldonium they contain.

These properties allow the use of Mildronate and Cardionate:

  • to improve metabolic processes in the body;
  • expanding the lumen in the vessels and improving blood flow in them;
  • slowing down necrotic changes in cells and tissues;
  • shortening the rehabilitation period after illness;
  • improving contractility of the heart muscle;
  • increasing the body's endurance under increased physical or mental stress;
  • increasing cellular immunity;
  • treatment of ophthalmological diseases.

Despite the fact that meldonium-based drugs have been recognized as doping agents by the World Anti-Doping Agency (WADA) since 2016, many experts categorically disagree with this.

Meldonium does not stimulate the athlete’s physical activity in any way, but only promotes the body’s adaptation to increased physical stress and ensures recovery after heavy training.

The drugs have good bioavailability (up to 80%) and are excreted from the body through the kidneys.

Indications for use

The medicine is used to regulate problems with metabolism, as well as for retarded mental and physical development in childhood, decreased ability to work, and physical and mental stress.

As a component of combination treatment, Cardonate is useful for chronic heart failure , coronary heart disease , metabolic cardiopathy , withdrawal syndrome , hyperthyroidism , cirrhosis , osteoporosis , peripheral neuritis , radiculopathy , iron deficiency and B12 deficiency anemia , bronchial asthma , cerebrovascular accident, hepatitis , hepatic and renal failure , bone fractures, neuralgia . In addition, for conditions requiring normalization of carbohydrate and lipid metabolism.

The medicine is also prescribed for gestosis in pregnant women, asthenic syndrome , fetoplacental insufficiency , rehabilitation after somatic and infectious diseases, intrauterine growth retardation, the need to prevent and treat neuropathies and encephalopathies that are caused by the use of anti-tuberculosis GINK group .

Undesirable effects

Both drugs are not without side effects, which are observed in rare cases and are usually not life-threatening for the patient.

Mildronate and Cardionate can cause:

  • tachycardia;
  • sudden changes in blood pressure;
  • nausea, belching, heartburn;
  • allergic reactions expressed in skin redness, rashes, swelling, etc.;
  • severe overexcitation;
  • headache.

Taking meldonium-based drugs immediately before bed can lead to insomnia

Injecting drugs into a muscle is fraught with the appearance of irritations on the skin and local inflammatory processes accompanied by pain.

An overdose of Mildronate and Cardionate is possible only when administered by injection. It manifests itself as: low blood pressure, headaches, dizziness, tachycardia, weakness.

If necessary, Mildronate and Cardionate are replaced by Vasopro, Vasonate, Metamax, Metonate, Mildrocard, Riboxil, Trizipine, Meldonium and other drugs.

There are practically no differences between Mildronate and Cardionate. However, the use of the former is in most cases recommended as a tonic for the body, while the latter has a better effect in maintaining the cardiovascular system.

In any case, the right to choose each prescribed drug remains with the doctor.

Instructions for use of Cardonate (Method and dosage)

The instructions for Cardonat indicate that adult patients and children over 15 years of age should take one capsule 2-3 times a day before or after meals. Reviews of Cardonate

Reviews of Cardonate are left by both those who have the diseases described in the indications and those who simply wanted to normalize their metabolism with its help. So, it is often used by women who want to change their body weight. They note that after a course of this remedy they began to feel much better and achieved the desired results. However, there are those for whom the drug did not help.

Bodybuilders also use it to maintain a good metabolic rate.

Reviews of Cardonate for children are rare. Parents generally note its benefits. However, it is important not to have exaggerated expectations for this product.

Contraindications

Both drugs cannot be used in the same cases, which experts include:

  • individual intolerance to meldonium or additional ingredients;
  • increased intracranial pressure due to the presence of intracranial tumors or impaired venous blood flow;
  • the period when a woman is carrying a child and breastfeeding it (experts do not have reliable data on the possible harmful effects of drugs on the child’s body);
  • the age category of patients under eighteen years of age (in this case the prohibition is not categorical);
  • swelling of unknown origin.

Special caution when using meldonium is required:

  • with pathological changes in the liver and kidneys (after all, it is with the help of these organs that drugs are metabolized and removed from the body);
  • patients of the older age category (in old age there may be chronic diseases that prevent the use of meldonium).

In the case of combined use of drugs based on meldonium with Nitroglycerin, Nifedipine, adrenergic blockers, tachycardia and hypotension may occur

If there are relative contraindications, medications should be taken in dosages recommended by a doctor and under his constant supervision.
